Dear URAC Accredited Company:
The purpose of this letter is to provide URAC accredited organizations, as well as those organizations with an accreditation review that are in-process, with current information regarding the URAC monitoring program. The monitoring program consists of the ‘Annual Corporate Attestation and Organizational Change Update” and a monitoring onsite visit.
The “Annual Attestation and Organizational Update” must be executed by the Chief Executive Officer each year. The attestation states that the organization is maintaining continued compliance with URAC standards as well as informing URAC of organizational changes. The attestation is attached and all accredited organizations must complete and return the attached attestation to URAC no later than September 30, 2007.
The monitoring program is designed to assess an accredited organization's continued compliance with URAC standards during their accreditation cycle. The monitoring onsite also provides the organization with the opportunity to discuss any questions pertaining to the standards and opportunities for enhancement prior to reaccreditation. Accredited organizations may be selected for an onsite monitoring visit at anytime during the accreditation cycle. Selection is pursuant to the terms of the Applicant Agreement between your organization and URAC. An organization will not incur any charges for monitoring onsite visits.
It is important to note that a monitoring onsite visit is not the same as an onsite visit conducted for cause such as when URAC has received a grievance or complaint about an accredited organization.
Accredited organizations selected for a monitoring onsite will receive notice 1 to 5 business day(s) prior to the monitoring onsite date via email to the primary contact person on the application. Please note that it is extremely important that URAC has the current contact information on file as we will assume that a non-response is an affirmative response and URAC will proceed with the onsite visit as intended. It is possible that a multi-site or multi-accredited organization may have more than one site or program selected for a monitoring onsite. The monitoring onsite is intended to minimize disruption of the organization's daily operations. Typically, the review will include review of case records, personnel credentials, training records, complaints, and quality management documentation. All information reviewed during the monitoring onsite will remain confidential. A brief summary will be provided to the accredited organization following the monitoring onsite. In the event non-compliance is found, the organization will be notified in writing and a compliance action plan may be developed by URAC to be implemented by the organization.
