URAC developed its Patient Centered Health Care Home (PCHCH) Programs to educate and guide health care practices, and/or their sponsoring health plans, insurers, and pilot programs in transforming into truly patient centered health care homes. URAC’s PCHCH Practice Achievement Program evaluates practices on evolving levels of success across 28 key URAC PCHCH Standards.

Meeting Materials:
Prior to the workshop you will need to log into your My Meeting registration account to access the workshop documents under the Upcoming Meetings section. Print out the materials and documents, and bring them with you to the workshop.


This complimentary webinar will:
    1. Introduce attendees to the PCHCH Programs
    2. Provide information about the PCHCH Practice Achievement and the two recognition levels:
      • PCHCH Achievement
      • PCHCH Achievement with Electronic Health Records (certified EHR and standards aligned with Meaningful Use)
    3. Identify key steps to get you started

Cancellation and Other Policies:
URAC reserves the right to cancel any Workshop with less than five (5) registrants and Webinar with less than two (2) registrants 14 days in advance of scheduled event. Additionally, URAC reserves the right to modify the program agenda based on number of registrants. Please review the agenda for your related event, if applicable.

ALL REGISTRATION FEES ARE NON-REFUNDABLE. Please check with your URAC Account Manager to verify if you are eligible for free registration prior to making registration payment. Registration cancellations must be made in writing to education@urac.org at least one week in advance of the program in order to receive a registration credit. Credits must be used by yourself or another employee within your organization to attend a future URAC educational program within one year of the cancellation date. Registrations are not approved until full payment is received.

Check payment should be sent with a copy of the invoice to URAC, Education Department, 1220 L Street, NW, Suite 400, Washington, DC. Access to workshop, webinar and conference materials will not be granted until payment is received in full.
