Patient Centered Health Care Home Practice Achievement for Practices Workshop
Thursday, April 18, 2013 (8am - 3pm US/Eastern)
This education workshop is offered to practices seeking URAC PCHCH Practice Achievement.
Note: If you are an auditing organization, you should register for the PCHCH Auditor Certification Workshop or PCHCH Achievement for Auditors Workshop.
URAC PCHCH Practice Achievement program evaluates practices on 28 key URAC PCHCH Practice Standards. These standards are considered essential elements of a patient centered health care home and include 7 mandatory standards. The essential standards align to the Joint Principles of the Patient Centered Medical Home and address key requirements for all Meaningful Use requirements for electronic medical records, e-prescribing, communication portal, and quality data submission.
URAC’s PCHCH Practice Achievement offers practices two levels of recognition:
- Achievement with Electronic Health Records (EHR)
- PCHCH Practice Achievement Standards for Practices / Auditors
- Interpretative Information
- Documents for Review and Preparation
- PCHCH Certified Auditor Roles, Responsibilities and Training Requirements
At the conclusion of this workshop participants will be able to:
- Recognize the essential building blocks of a patient centered health care home and URAC PCHCH Programs
- Describe the URAC Patient Centered Health Care Homes (PCHCH) Practice Achievement Programs - Understand PCHCH Achievement Program standards requirements
- Distinguish mandatory achievement standards
- Identify PCHCH Practice Achievement EHR standards

Cancellation and Other Policies:
URAC reserves the right to cancel any Workshop with less than five (5) registrants and Webinar with less than two (2) registrants 14 days in advance of scheduled event.
ALL REGISTRATION FEES ARE NON-REFUNDABLE. Please check with your URAC Account Manager to verify if you are eligible for free registration prior to making registration payment. Registration cancellations must be made at least one week in advance of the program in order to receive a registration credit. Credits must be used by yourself or another employee within your organization to attend a future URAC educational program within one year of the cancellation date. Registrations are not approved until full payment is received.
Check payment should be sent with a copy of the invoice to URAC, Education Department, 1220 L Street, NW, Suite 400, Washington, DC. Access to workshop, webinar and conference materials will not be granted until payment is received in full.

This continuing nursing education activity has been approved for 6.5 contact hours by the Virginia Nurses' Association Continuing Education Approval Committee, an accredited approver by the American Nurses Credentialing Center's Commission on Accreditation.
