Thursday, July 24th, 2014

Hospitals and health systems are shifting focus from treating patients who need immediate care to improving the health of their community at large. This approach is called population health management (PHM), which produces better outcomes and helps healthcare organizations deliver higher quality care with increased profitability.

ADVANCE Events is hosting an online conference on September 10, 2014, called A Roadmap for Population Health Management, to help healthcare executives and professionals learn about successful PHM strategies that are currently being used. All healthcare professionals are welcome to register for free at www.advanceweb.com/events.

This free event is sponsored by three organizations leading the way in health management: Applied Health Analytics, Thomas Jefferson University and URAC.
